APPLICATION FORM FOR DATA OWNERS TO USE THEIR RIGHTS

In the Personal Data Protection Law No. 6698 (“KVK Law”) to personal data owners defined as relevant persons (“here in after “Applicant") personal data in accordance with Article 11 of the KVK Law

The right to make certain requests regarding processing is granted.

In accordance with the first paragraph of Article 13 of the KVK Law; Applications regarding these rights to our Company, which is the data controller, must be made in writing or to the Personal Data Protection Board (“Board") must be forwarded to us by other methods determined by us.

In this context, applications to be made to our Company in writing must be made by printing this form;

• By personal application of the Applicant,
• through a notary,
• The Company is registered by signing with the secure electronic signature defined in the Electronic Signature Law No. 5070 by the Applicant. coniasoft@hs01.kep.tr by sending to your e-mail address,
• It can be forwarded to us by e-mail sent by the Applicant from the e-mail address previously notified to our Company and registered in our Company's system.

Below, information is given specific to written application channels on how written applications can be submitted to us.

Our company may request additional information and take the necessary measures to verify the identity of the Applicant in applications made by e-mail. In addition, after the other methods determined by the Board are announced, our Company will announce how applications will be received through these methods. Your applications submitted to us will be responded to within thirty days from the date your request reaches us, depending on the nature of the request, in accordance with the 2nd paragraph of Article 13 of the KVK Law. Our responses will be delivered to you in writing or electronically in accordance with the provision of Article 13 of the relevant KVK Law.

In order to prevent legal risks that may arise from illegal and unfair data sharing and especially to ensure the security of your personal data, our Company reserves the right to request additional documents and identification (copy of identity card or driver's license, etc.) for identification and authorization determination. If the information regarding your requests submitted within the scope of the form is not accurate and up-to-date or an unauthorized application is made, our Company does not accept responsibility for requests arising from such incorrect information or unauthorized application.
